1. Acknowledgment of Risk
I understand that participation in activities with the Coleman Youth Association involves inherent risks. These activities may include, but are not limited to:
I understand that these activities may take place at facilities owned or operated by third parties that CYA uses for games or practices.
I acknowledge that sports and athletic activities involve risks of injury, including serious injury, permanent disability, or death. These risks may result from falls, contact with other participants, equipment, field or facility conditions, weather, or the actions of others.
2. Assumption of Risk
I voluntarily choose for myself or my child to participate in CYA activities. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of those released below.
3. Release and Waiver of Liability
In consideration of being allowed to participate, I, on behalf of myself, my child, and our heirs, representatives, and assigns, release and hold harmless:
The Coleman Youth Association
Its directors, officers, coaches, volunteers, and representatives
Any schools, churches, cities, counties, or other entities that own or operate facilities used by CYA
Any sponsors or partners of CYA
from any and all claims, demands, causes of action, damages, or liability arising out of or related to any injury, illness, loss, or damage that may occur during or as a result of participation in CYA activities or use of related facilities, including claims based on negligence.
4. Medical Authorization
I authorize CYA staff, coaches, or volunteers to obtain emergency medical treatment for the participant if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
5. Photo and Media Release
I grant permission for the Coleman Youth Association to use photographs or video of the participant for promotional, social media, or informational purposes, without compensation.
6. Code of Conduct Acknowledgment
I understand that participation may be revoked if the participant or parent/guardian engages in unsafe, disruptive, or unsportsmanlike behavior.
7. Agreement and Understanding
I have read this waiver carefully and fully understand its terms. I understand that I am giving up substantial rights by signing it, and I sign it freely and voluntarily.
Once teams have been formed, there is a no refund policy.
